DESCRIPTION

Position Summary The Registered Dietician role will develop trusting partnerships with Wesley Health Center (WHC) patients. They will learn about patient health concerns, goals, and dietary restrictions to work with patients to lead healthier lives. They should be analytical, caring, knowledgeable with good communication, active listening, and problem-solving skills.

Essential Duties and ResponsibilitiesTo perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Provides and manages direct patient care, including assessments, evaluation, and treatment for nutritional, dietary, and feeding risks or disorders.
  • Consults with providers regarding patients’ diagnosis, prognosis, and progress.
  • Develops and implements functional, measurable goals to address nutrition needs or dietary changes.
  • Reassesses patient progress on goals and need for continued services on a regular basis.
  • Assists in the identification, assessment, and intervention for patients referred to and/or in need of nutrition services/evaluation or diagnosed with nutrition related disease.
  • Ensures patients and families receive accurate explanations of nutritional assessments and goals for needed changes or maintenance of health.
  • Develops services that address the functional needs of the patient to improve health and development in a manner that reflects understanding of family dynamics that affect patient health.
  • Instructs patient and/or family members in the proper and safe usage of supplements, formulas, diets, exercise, and activity.
  • Assists in coordination of services and recommends appropriate referrals as needed.
  • Collaborates with community agencies and professionals to provide timely and seamless services to eligible patients and families.
  • Develops and maintains patient progress reports and documentation.
  • Provides consultation and timely reports of patient evaluation, reassessments, and disposition to providers.
  • Participates in specified nutrition promotion, education and/or prevention programs for associated nutritional health issues as appropriate. • Participates in the Quality Improvement Program through incident reporting and identifying opportunities for improvement.
  • Participates in documentation reviews.
  • Assists in planning and coordination outreach efforts and /or other related programs or projects.

Other Duties:

  • Be familiar with Federal and State Rules, Regulations and Practices in a Federally Qualified Health Center.
  • Be familiar with Federal and State Health Care Program requirements and assist in carrying out these programs.

  • Ability to relate to the public regardless of ethnic, religion, sexual orientation, or economic status.
  • Must be willing to travel to other WHC locations.

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education/Experience:

  • Current valid registration as a Registered Dietitian. • Bachelor’s degree in dietetics, foods and nutrition, clinical nutrition, public health nutrition, or a related field.
  • Any combination of education and/or experience that provides the necessary skills may be considered.
  • Participate in continuing education.

Language Ability:
Read, analyze and interpret business, professional, technical or governmental documents. Write reports, business correspondence and procedure manuals. Effectively present information and respond to questions from managers, customers and the public.

Mathematical Ability:
Add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Compute rate, ratio and percent, and draw and interpret bar graphs.

Reasoning Ability:
Apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Deal with problems involving several concrete variables in standardized situations.

Computer Skills:
Electronic Health Record (EHR), MS Office, Excel, PowerPoint, Internet Explorer.
Certificates and Licenses:
• Current CPR certification.
• Driver's License and Proof of Insurance may be required if requesting mileage reimbursement.
• Participates in continuing education programs to maintain professional competence and skills.
• Valid Fingerprint Clearance Card.
• Bilingual in Spanish and English preferred.

Public Speaking: Ability to present facts about diet and nutrition during community events and speaking engagements to promote excitement about healthier eating habits.
